What is Personal Information and why do we collect it?

Personal Information is information or an opinion that identifies an individual. Examples of Personal Information we collect include: names, addresses and Medicare details
This Personal Information is obtained and may be discussed in ways including (yourself, your GP, carers, case managers, relevant care facility or via our website)
We collect your Personal Information for the primary purpose of providing our services to you, providing information to our clients and marketing. It is an important to us that your Personal Information is up to date. We will take reasonable steps to make sure that your Personal Information is accurate, complete and up-to-date. Scheduling a Home Visit

Given the nature of home visits, it’d be near impossible to book an appointment online in a timely manner. As such, once you submit this form with your preferred date and time/s, we will contact you to confirm your booking. Until you receive a call or email confirming your home visit, the home visit is not confirmed.

Payment

Home visits are charged a little differently to our clinic appointments. This is to address costs associated with things like admin travel and time. To see our schedule of fees, click here.

Attendance

Once a home visit has been booked in and confirmed, it is your responsibility to ensure you are home at the reserved time. In the event you or the client resides in a gated community, critical care unit or other such location that requires security access, you are responsible to inform the necessary staff of the location the appointment is taking place.

Home Visit Disclaimer

Whilst we provide home based services to you, your main contact will remain your GP or Case manager
The Toe truck is open during standard business hours Monday to Friday and does not provide emergency services or any services outside the scope of a Podiatrist. Our scope during home visits are limited to both the Health professional and clients’ safety, best practice, and the environment of your home.
In any case of a medical emergency, your first point of call would need to be to Emergency Services or GP.

Cancellation Policy

We know your time is valuable, the same as our Podiatrist’s time. Thus we would require at least 12 hours’ (in-clinic) or 24 hours (Home visit) notice if you cannot keep your in clinic appointment so that we can give your appointment time to another patient who can attend.
A cancellation fee of $30 applies to non-attendance or late cancellations. The fee is not rebatable through Medicare.
If there are any circumstances beyond your control which prevent you from cancelling your appointment, please let us know as soon as possible.

PRE HOME VISIT RISK & SAFETY ASSESSMENT

In order to send one of your Health Professionals our to our clients home’s it is important that you confirm that;

  • You consent to one of our staff members visiting your home
  • You provide us with any special instructions to access your home and preferred mode of entry.
  • If someone else will be present during our visit
  • That if you do have pets in the house or garden, where the health professional cannot preform the treatment safely the pet is placed in another room during the visit.
  • Nobody will be smoking in the house during the visit
  • You will inform us of any significant alcohol or drug consumption in the house, if any before our visit.
  • If anyone in the house has a history of aggression
  • If there are weapons or firearms in the house, that they are stored to the standard provided for by the Firearms Act 1996 ( Schedule4).
  • If anyone in the house recently suffering from an infective illness? (eg flu, scabies, chicken pox, TB, norovirus gastroenteritis).
  • There are no other factors that might make it difficult for a clinician to conduct a treatment at home? For example, suitable space or lighting available to conduct the treatment?
  • You consider it safe for a clinician to conduct a visit to this client’s home
COVID-19 RISK ASSESSMENT

Nobody in the home has been diagnosed with COVID-19, or awaiting test results for COVID-19? i.e confirmed case or high risk for potential case
Have not been overseas or on a cruise ship in the last 14 days
Have not been in contact with, or do you live with, someone who has COVID-19, or waiting test results for COVID-19? i.e contact with a confirmed case or high risk for potential case
Have not had any symptoms such as fever, night sweats or chills, or respiratory symptoms such as runny nose, cough, sore throat or shortness of breath?
You will remain vigilant for the development of fever or flu-like symptoms as well as avoid contact with anyone who is unwell or has recently travelled overseas.
To practice safe social distancing, please avoid having any visitors and limit the number of family members within the home when your health professional attends.
Clinicians coming to see you at home will be continuing to you these questions prior to entering your home. If you have any questions or concerns please do not hesitate ask us, your care coordinator, or other medical professionals involved in your care.
Services will continue as per usual for now.
Be aware this is a changing environment and we will update you as recommendations change.
Education for clients; To prevent infection please practice good hand hygiene and respiratory hygiene by:

  • Practicing excellent hand hygiene
  • Avoiding Touching your face
  • Maintaining >1.5m distance from client/therapist at all time where possible
  • Notifying your therapist & Care Coordinator as soon as any changes in symptoms or contact arise
